WARRANTY: NORTHWEST RESTORATION WARRANTIES ALL WORK PERFORMED BY ITS EMPLOYEES AND/OR
SUBCONTRACTORS TO BE FREE FROM DEFECTS FOR A PERIOD OF ONE (1) YEAR FROM THE DATE OF COMPLETION.
THIS WARRANTY IS THE EXCLUSIVE WARRANTY AND APPLIES ONLY TO THE WORK PERFORMED BY NORTH WEST
RESTORATION. THIS WARRANTY SHALL NOT BE VARIED BY COURSE OF DEALING OR USAGE OF TRADE OR VERBAL
ASSERTIONS OR ACTS. THIS WARRANTY SHALL NOT APPLY TO ANY DEFECTS IN ANY MATERIALS USED BY NORTH WEST
RESTORATION OR ITS EMPLOYEES AND/OR SUBCONTRACTORS WHO PERFORM THE WORK. NORTH WEST RESTORATION'S
ONLY OBLIGATION UNDER THIS WARRANTY SHALL BE TO REPAIR OR REPLACE, AT ITS OPTION, ANY DEFECTIVE WORK
PERFORMED PURSUANT TO THE CONTRACT BETWEEN NORTH WEST RESTORATION AND THE OWNER OF THE PROPERTY
ON WHICH THE WORK HAS BEEN DONE. UNDER NO CIRCUMSTANCES SHALL NORTH WEST RESTORATION BE LIABLE FOR
ANY ACCIDENTAL, INCIDENTAL, OR CONSEQUENTIAL DAMAGES RESULTING FROM ANY DEFECT COVERED BY THIS
WARRANTY.
Chapter 64.50 RCW contains important requirements you must follow before you may file a lawsuit for defective
construction against the seller or builder of your home. Forty-five days before you file your lawsuit, you must deliver to
the seller or builder a written notice of any construction conditions you allege are defective and provide your seller or
builder the opportunity to make an offer to repair or pay for the defects. You are not obligated to accept any offer made
by the builder or seller. There are strict deadlines and procedures under state law, and failure to follow them may affect
your ability to file a lawsuit.
TERMS AND CONDITIONS: 100% of contract amount due upon completion of work.
The owner upon signing this agreement represents and warrants that he is the owner of the aforesaid premises and that he
has read this agreement. Unless otherwise provided by agreement between the parties, the insured/owner agrees to pay a
service charge of 1.5% per month (annual rate 18%), and a $50.00 re -billing fee on delinquent balances. In the event the
insured/owner wishes to cancel this contract, notification must be given to North West Restoration within 24 hours of signing
to avoid a penalty of exactly 10% of the contract amount. A $100.00 handling fee will be charged for any returned checks. The
Insured/owner agrees to keep premises insured during restoration. This contract binds the insured/owner, the
insured/owner's successors, and assigns. Any action to enforce the Terms and Conditions of this contract shall occur in
Benton County, Washington. Which action shall award all attorney's fees and costs to the prevailing party. The insured/owner
agrees to pay all collection fees and costs, including attorney's fees for all demands, notices, liens and other actions necessary
to enforce the Terms and Conditions of this contract.
NOTICE TO CUSTOMER: This contractor, North West Restoration FSWW is registered with the State of Washington,
Registration No. NORTHWR033DL, as a general/specialty contractor and has posted with the state a bond or cash deposit of
$12,000/$4,000 for the purpose of satisfying claims against the contractor for negligent or improper work or breach of
contract in the conduct of the contractor's business. The expiration date of this contractor's registration is 02/15/22. This
bond or cash deposit may not be sufficient to cover a claim, which might arise from the work done under your contract. If any
supplier of materials used in your construction project or any employee of the contractor or subcontractor is not paid by the
contractor or subcontractor on yourjob, your property may be liened to force payment. If you wish additional protection,
you may request the contractor to provide you with original "lien release" documents from each supplier or subcontractor on
your project. The contractor is required to provide you with further information about lien release documents if you request
it. General information is also available from the Department of Labor and Industries.

Indemnification AGREEMENT: covin -19
The customer understands that the Covid-19 virus is an unknown and unpredictable
virus. NWR will use the best practices and current industry standard and is committed to
the most positive application process of disinfectants at this time. There is no guarantee
expressed or implied that this process will stop the spread of Covid-19.
1. 1 release, Hold Harmless and indemnify North West Restoration, their employees,
officers, directors, sub -contractors and agents (collectively "NWR") from any and all
claims.
2. If/ when disinfectant has been applied to agreed upon areas, transportation vehicles
and/or buildings, NWR is not responsible or liable for any contamination after disinfecting
has been completed.
3. 1 agree to release, hold harmless and indemnify NWR from any and all claims, including
attorney's fees or damage to my personal property, which may occur as a result of my
choice to forgo the actions recommended by NWR. If NWR incurs any of these types of
expenses, I agree to reimburse NWR.
