In order to comply with ADA requirements, it was necessary to install pedestrian push button extensions on the 4 signal poles at Lewis Street and Oregon Avenue. Some concrete adjacent to driveways was 4" in thickness per the plans and large truck exiting and entering the business cut the comers with large, heavy loaded trailers damaging the sidewalk. It was necessary to replace the substandard sidewalk with thicker concrete (6") in order to elevate Mure damage. The monument at James Street ended up in the splitter island planter and wasn't addressed on the plans to be raised to the new grade (plans noted to protect only). The light pole at East Marvin Street and Oregon Avenue was installed per the plans and too close to the overhead power (FPUD made us aware of this problem) so it was necessary to move the pole 1 O' to the south along Oregon Avenue. In order to comply with USPS Postmaster's requirements when reinstalling the relocated mailboxes, we had to install a lockable cluster mailbox per their standard specification. Due to damage caused by a truck, it was necessary to repair the curb/gutter and landscaping (including replacing 2 trees). Unfortunately, the offending truck driver was not caught so no restitution will be collected to pay for this damage. At 1115 North Oregon Avenue (Thermo-King) the landscape restoration wasn't noted on the plans or specification to be restored. This required a major re-work to their irrigation system and sod restoration. • Page1
