made and that the compensation for this Change Order is full and complete and is the only compensation
due or owing for this Change Order. Further, it is mutually agreed that this document will supplement the
present Contract Documents and that the provisions of the previously executed Contract Documents shall
apply to this Change Order.

Change Order Proposal
COP 012 — Oil Soill Vault
Background Information: Sheet E-3 of the Contract Plans requires the Contractor to install a
"Utility Service Oil Spill Vault as Required by FPUD." Franklin PUD has decided to waive this
requirement because the distance from the transformer to the river is over 200 feet.
Cost Evaluation: The vault was already on site and was returned to the supplier. The credit,
including labor, equipment, and the vault (minus shipping and restocking), is shown below.

COP 014 —Additional Pavino in Court Street
Background Information: The installation of the raw water line and the duct bank require
removal and restoration of asphalt pavement in Court Street. The Contract Documents require
restoration for the trenching associated with the duct bank and raw water line. As the
Contractor installed the utilities, it encountered several additional utilities which expanded the
roadway area to be removed and replaced. Additionally, there were several small pockets of
existing pavement which could have remained in place but would have made the roadway a
patchwork of new pavement and old pavement. The Contractor proposed to remove the
patches of existing pavement and install one new, continuous section of pavement.
Cost Evaluation: The cost includes the additional removal of asphalt, the additional placement of
crushed surfacing, and the additional HMA tonnage required to expand the paving area to
include the entire roadway from the pump station to the treatment plant driveway. The cost of
the change order proposal, combined with the paving cost included in the Contractor's Schedule
of Values, appears to be a reasonable sum to complete this amount of street restoration.

COP 015 — Man Gate Addition
Background Information: The City's electronic sliding gate at the treatment plant can cause
issues for personnel accessing the plant. The Contract Documents for the pump station
building also include a sliding gate. The City expressed an interest in adding a man gate with a
physical locking mechanism which can be used to access the site if the electronic sliding gate is
malfunctioning. Also, the fencing in the south corner of the property was adjusted and required
an additional fence post and panel.
Cost Evaluation: The Contractor offered to add a 48 -inch wide custom made gate to match the
appearance of the fencing. The price includes the installation of two additional posts, gate
hardware, and welding of the panel to the uprights. It also includes the additional post and
panel in the south corner of the property.

COP 016 — CP -100 Modifications
Background Information: During bench testing of the panel, it was noted that the mounting
height of the equipment is not suitable for operators to read the interface. The City has
requested a new door or skin cover on the panel with the equipment situated at a suitable
height.
Cost Evaluation: The cost includes the labor and materials to install a new door and rearrange
the equipment.

COP 017 — HVAC Panel
Background Information: During testing of the panel, it was noted that modifications to the
design are necessary for the panel to perform as needed by the City. The Contractor needs to
rewire the panel.
Cost Evaluation: The cost includes the labor and material to modify the panel as directed and
appears to be reasonable.

COP 018 — Fire Extinguisher
Background Information: The fire marshal noted that four fire extinguishers are required for the
pump station. The Contract Documents require three fire extinguishers, so one additional fire
extinguisher must be provided.
