Further, it Is mutually agreed that this document will supplement the present Contract Documents and that the provisions of the previously executed Contract Documents shall apply to this Change Order. AG EED TP AND ACCEPTED: CONTRAVOR Date APPROVED: CONSTRUCTION MANAGER Date Memo Public Works Department Engineering Division To: Kent McCue, Construction Manager CC: Ryan Mahaffey, Construction Inspector Brenda Timpany, Admin Assistant 1 From: AndreyAvetisyan, P.E., Civil Engineer Date: July 20, 2015 Re: Change Order #3, Project No. M3 -OV -3R-15-01, 2015 Overlays Description of Change: This change order provides additional re -mobilization fees for milling and paving contractors to come back from out of town to complete the softball parking lot overlay work due to an unforeseen scheduling conflict with a state soccer tournament. During pre -construction meeting, the Contractor was made aware of the 4"' of July activities in that parking lot vicinity and were advised to either have all of that parking lot work completed or not started until after the 4"'. Contractor chose to re- submit a modified schedule in order to get that parking lot completed before the &h. Unfortunately, the work could not be done as scheduled due to a previously scheduled state soccer tournament that was prearranged to occur during same week as the construction activities. This change altered the Contractor's scheduling with other previous commitments and they agreed to bring the crews back after the 0 of July at an additional cost. Entitlement: This work is not included in the original Contract. The Contractor is entitled to added compensation for added work performed in accordance with Section 1-04.4. No entitlement is found for a revision to the number of working days. Adding this work will not increase the time to complete all critical items of work. In accordance with Section 1-08.8, no revision to the number of working days will be made by this change order. Price: A lump sum price was received from the Contractor detailing their cost for this work. The Contractor included costs re -mobilization for the milling ($1800) and paving ($2000) crews from out of town in their determination of the final price. The City of Pasco Construction Manager reviewed and accepted the proposed cost. The total cost for this change order work is $3800. 0 Page 1
