I From: Teresa Reed -Jennings PE, Sr. Civil Engineer Date: July 5, 2016 RE: Project #{CP7SE-1 A-16-01 - Kahlotus Lift Station Force Main Pipe Change Order No. 1 ' •1 Public Works ' Engineering Change Order No. 1 reflects the following revisions to the project listed below. This change order was requested by the City. When the Contractor performed his required field exploration prior to beginning construction, it was discovered that the location of the existing 4 -inch and 8 -inch wastewater force mains differed from the location shown on the design plans. Depiction of the existing force mains employed the previous force main design plans and the painted field locates picked up by the surveyor. The required field redesign relocated the new 12 -inch force main from the west side of the existing force mains to the east side, putting the new pipe closer to Commercial Avenue. C & E Trenching provided a cost breakdown for the extra work associated with the redesign change order items listed below: + The tie-in connection to the existing 12 -inch force main located under US 395 required a fusion welded flange and an additional fitting. * Pipe installation along Commercial Avenue required additional sawcutting, removal, and disposal of asphalt. * The City replaced the recently removed section of asphalt with a 6 -foot wide gravel shoulder. The paved width of the southbound Commercial Avenue lane remains 12 to 13 feet wide. * The ARV vault associated with the relocated force main ended up closer to Commercial Avenue and required guard posts. Additionally, this change order. * Reconciled various bid item quantities to reflect installed quantities (subtract values). « The tracer wires for the existing force mains under US 395 were taped to the marker posts so the Contractor was asked to install another tracer box to properly house the wires. The pre-tax cost associated with this change order is $10,636.02.
