It is mutually agreed by both parties that this Change Order fully describes the change(s) that is (are) being
made and that the compensation for this Change Order is full and complete and is the only compensation
due or owing for this Change Order. Further, it is mutually agreed that this document will supplement the
present Contract Documents and that the provisions of the previously executed Contract Documents shall
apply to this Change Order.

Description of Chanoe:
This change order adds work to the Contract to crack seal five (5) additional cul-de-sacs within
work areas that were not included in original contract (see attached maps for locations). The first
three cul-de-sacs are on the south side of Antigua Drive in Area 7 of the project. These three were
unintentionally excluded from the original contract due to lines on map being near street name
lettering. The other two cul-de-sacs are located on the north side of Meadow Beauty Drive. These
two did not even appear on our street base map at all. With the intent of crack sealing being done
by areas, it is beneficial to include these added quantities to this contract by change order and not
have to return to these areas at a later time just for the minor work.
Entitlement:
This work is not included in the original Contract. The Contractor is entitled to added compensation
for added work performed in accordance with Section 1-04.4.
No entitlement is found for a revision to the number of working days. Adding this work will not
increase the time to complete all critical items of work. In accordance with Section 1-08.8, no
revision to the number of working days will be made by this change order.
Price:
A lump sum price was received from the Contractor detailing their cost. The Contractor based the
price on a field measurement of 1548 linear feet of additional cracks at a rate of $0.56 per linear
foot. The City of Pasco Construction Manager reviewed and accepted the proposed cost, which
includes all labor, equipment, time and material.
The total cost for this change order work is $866.88.
