It is mutually agreed by both parties that this Change Order fully describes the change(s) that is (are) being
made and that the compensation for this Change Order is full and complete and is the only compensation
due or owing for this Change Order. Further, it is mutually agreed that this document will supplement the

Re: Change Order #6, Road 68 Improvements Phase 2 and Interconnect Rd 68 Signals
Change Order No. 6 reflects the revisions to the Road 68 Improvements Phase 2 Project listed below. This
change order was requested by the City.
1) The installation of the new center curb medians in Rd 68 prevents drivers from being able to go
straight or turn left out of the drivewayjust north of Applebee's on Rd 68. The original project didn't
address changing the existing striping within the driveway to eliminate the straight/left turn arrow.
The straight/left turn arrow needs to be removed and a right turn arrow needs to be installed to
prevent driver confusion when existing the driveway.
The Following Table shows the costs associated with Change Order #6.

The Contractor submitted a lump sum breakdown for this work. Based on that breakdown the striping will
cost approximately $2.83/LF which is much higher than their original bid item price of $0.75/1-F. When
comparing this to the Ast & 1 s Ave RR Crossing project completed last year which had a unit price of
$1.15/1-F the price was still found to be high. We have determined that this extra expense is likely due to the
small quantity of work and the contractor needing to remobilize the site to complete the work. Considering
those items the price was found to be acceptable.
