This site visit revealed that since the final walk through for the project on October 3, 2014 significant damage had been done to the north east portion of the project located on the north edge of a driveway entrance into the Lakeview Trailer Court. The residents of the Lakeview Trailer Court have been driving into the newly reshaped gravel ditch over the new catch basin when exiting the driveway. This has caused the ditch to lose its shape, and fill the catch basin with gravel and not function as designed. See photos below of the area during construction and on October 14, 2014. September 26, 2014 October 14, 2014 At the time of the final walk through the ditch was bowled at the catch basin to force water into the catch basin instead of draining across the roadway. On the day of our site visit, October 15, 2015, this shape to the ditch was no longer present due to the vehicles repeatedly driving over the ditch as shown in the above picture. In order to prevent the issues that existed prior to the construction of the project from re-emerging this area must be reconstructed. I recommend installing concrete curbing around the radius and lining the ditch with 0 Page 1 concrete immediately adjacent to the catch basin to prevent this issue from reoccurring once the contractor leaves the project site. Bollards may also be needed behind the curb to ensure vehicles do not drive around the curbing. Additionally, the residents of the Lakeview Trailer Court have been walking through the newly placed hydro - seed within the project limits. This will likely cause some areas of the hillside to erode sediment into the new ditches. The hydro -seeded areas will need to be monitored to see how the seed grows. If inadequate growth occurs, the vertical slopes may need additional erosion control measures to help reduce erosion due to wind and rain. 0 Page 2 Memo To: Mike Pawlak, P.E., City Engineer From: Kate Thompson, P.E., Civil Engineer Date: October 29, 2014 Re: Change Order #5, Rd 40 East Storm Drainage Public Works Department Engineering Division Change Order No. 5 reflects the revisions to the Rd 40 East Storm Drainage Project listed below. This change order was requested by the City to correct issues that developed after construction. 1) Residents at the Lakeview Trailer Court have been driving over the newly reshaped gravel ditch located on the north edge of the driveway entrance into the Lakeview Trailer Court. This has caused significant damage to the ditch, and is preventing the ditch from functioning properly. Please see the attached memo regarding the damage for additional information. In order to correct this issue we are installing additional concrete ditch in this location which will be approximately 6' wide by 11' long with 28' of curbing. The curbing will be 8" wide with 10" of reveal and will be painted yellow in order to keep vehicles from entering the new concrete ditch. This construction will require some asphalt to be patched back near the new concrete ditch. 2) The existing ditch upstream of the new concrete ditch described in item 1 above is eroding and depositing large amounts of sediment into the catch basin which will be located within the new concrete ditch. To protect the ditch from further erosion, we will be compacting and adding 4" of CSBC to the existing ditch line for a length of 200' north from the new concrete ditch. Within this new gravel ditch section rock check dams will be installed on 10' centers. These check dams will help slow the flow of the water and help sediment settle out before reaching the new catch basin. The Additional Concrete Ditch costs approximately $304.24/SY vs the original unit price for Concrete Lined Ditch which was $75.72/SY. The Additional Concrete Ditch costs more due to the addition of the painted curb, This price was found t}o�be�acceptable. • Page 1 The price for the Asphalt Patch was found to be approximately $91.07/SY which is slightly higher than the original unit price for Pavement Repair which was $72.82/SY. This price difference is likely due to having to remobilize the paving crew for a small area of work. This price was found to be acceptable. The price for the Gravel V -Ditch was the same as the original unit price for Gravel V -Ditch.
