AGREED TO AND ACCEPTED: CONTRACTOR Dale APPROVED' Com^ y F-1 l—1 alIL CITY ENGIN Date ER Dale APPROVED: PROJECT MANAGER Date ' APPROVED: -- P 10 PUBL Dale DIRECTOR Memo To: Mike Pawlak, P.E., City Engineer From: Kate Thompson, P.E., Civil Engineer Date: October 17, 2014 Re: Change Order #3, Rd 40 East Storm Drainage Public Works Department Engineering Division Change Order No. 3 reflects the revisions to the Rd 40 East Storm Drainage Project listed below. This change order was requested by the City. 1) Catch Basin #2 and Catch Basin #3 both have 12" pipes leaving the catch basin. The standard drawing requires that a 900 elbow be installed to act as an oil water separator on the exiting pipe. Due to its size, the 12" 90° elbow it doesn't fit inside the standard catch basins which have already been installed. The plans did not call out for these catch basins to be increased in size to a 48" barrel to accommodate the 12" 90° elbow. Due to this a low profile oil water separator will need to be installed in both of these catch basins. The table below lists all changes and costs associated for the Rd 40 East Storm Drainage project: NUMBER I ITEM UNIT UNIT PRICE QUANTITY TOTAL 1 900 Elbow EA -$145.29 2 -$290.58 2 Low Profile Oil Water Separator EA $326.07 2 $652.14 SUBTOTAL $361.56 TAX 8.6% $31.09 TOTAL COST CHANGE ORDER #2 $392.65 The Low Profile Oil Water Separator is a new item that is being made by the manufacturer (Raven) specifically to fit in Pasco's round 24" catch basins. This project will pilot installation of this item. Due to this, there are no previous projects to use as a check for the price submitted. It is likely that this price is high, due to the contractor's lack of familiarity with the installation of the product. The price will probably decrease in the future if we continue to use the items and contractors become comfortable with the installation. The 900 Elbows were part of the per each cost for Catch Basin Type 1 (Bid Item #14, $1,088.04/EA) in the original bid proposal. it I V7 lulk 0 Page 1 Katherine Thompson From: Jake Wray <jake@candetrenching.com> Sent: Thursday, October 16, 2014 4:04 PM To: Katherine Thompson Subject: RE: Change order for Catch basin Hoods for Road 40 east. Hi Jake, Can you send me the cost for the original 90's that were part of the total cost for the catch basins installed?
