SUBJECT: LOSS CONTROL; ACCIDENT INVESTIGATION; AND SAFETY COMMITTEE
L PURPOSE: Each employee of the City of Pasco has the responsibility to protect
City property from loss or damage. Managers and supervisors are also
responsible for assuring employees a safe and healthful work place. This
Administrative Order has been developed as a tool to help meet those
responsibilities and contains those policies and procedures that have been
developed to help minimize the City's exposure to loss.
H. RESPONSIBILITY: The implementation of these policies is mandatory throughout
the City. All policies contained in this Administrative Order supersede any
currently in existence. Each Department is responsible for informing its
employees of these policies and enforcing them.
III. LOSS CONTROL COMMITTEE: The Loss Control Committee is established to improve
communications between the Departments and the Risk Manager (City Attorney).
This communication is essential to assure that a number of changing exposures to
• loss are adequately protected.
A. Members. The Committee is composed of the following persons:
1. Permanent Members: Risk Manager (Chairperson); and Personnel Officer
(Chair Pro Tem).
2. Alternating Members (annual): Parks or Public Works Director; Police or
Fire Chief; and Community Development or Finance Director.
B. Duties. The Committee has the following duties:
1 . Discuss, evaluate, and recommend action(s) to protect the interests of
the City, its employees and citizens. This shall include the review of
City operations and capital improvements to see that they are
adequately protected from loss, discussion of losses, health matters
and the City's safety program.
2. Implement (with the approval of the City Manager) and monitor loss
control standards and procedures.
3. Review Incident Reports filed with the Risk Manager (see Administrative

C. Meetings and Minutes. The Committee shall meet at least monthly and will

IV. ACCIDENT INVESTIGATION:
A. Reporting. All injury and non-injury accidents with loss or damage to
City property and involving City employees shall be reported to the Risk
Manager on an "Incident Report form" (see Administrative Order No. 53)
within 24 hours of the accident. Accidents that cause serious injuries

• B. Investigation. Investigation of the cause of the accident shall be
conducted by the Risk Manager, Personnel Officer, the immediate supervisor
of the employee(s) involved, and employee representative if available, and
any other person with the special expertise required to evaluate the facts
relating to the cause of the accident selected by the Risk Manager. The
findings of the investigation shall be documented by the Risk Manager and
reported to the Safety Committee.
C. OSHA Reporting. The Personnel Officer shall record occupational injury
and illness information on forms OSHA-101-Supplementary Record
Occupational Injuries and Illness and OSHA-200-Log and Summary.
V. SAFETY COMMITTEE: AManagement-Employee Safety Committee shall be formed and
report to the Loss Control Committee,
A. Members. The Committee is composed of the following persons:

City Hall non-management employees; one employee elected from each of
the two departments whose department head is not serving as a Safety
• Committee member for that year (i.e., if the Fire Chief is serving, an
employee from the Police Department shall be elected).
B. Chairman. The Committee shall be chaired by one of the members elected by
the Committee.
C. Secretary. The Personnel Officer shall serve as Secretary to the
Committee and shall maintain the Committee's records, including agendas,
minutes, correspondence, etc.
D. Duties. The Committee shall meet and address the following as required by
WAC 296-24-045:
1 . A review of the safety and health inspection reports to assist in
correction of identified unsafe conditions or practices.
2. An evaluation of the accident investigations conducted since the last
meeting to determine if the cause of the unsafe act or unsafe
condition involved was properly identified and corrected. The Safety
Committee shall report its findings to the Loss Control Committee
which shall make the final deternination as to cause, if the
determination of the Safety Committee differs from the determination
made in the report of the Risk Manager.
3. An evaluation of the accident and illness prevention program with a
• discussion of recommendations for improvement when indicated.
4. The committee shall also function as a forum for the discussion of
potential or existing work place safety concerns or issues. The
committee may advise (in writing) the Risk Manager of a particular
safety concern identified by the committee, provided, however, the
committee shall first be assured that the affected department has been
informed of the particular safety concern and has not sufficiently
responded.
E. Means. The Committee shall be responsible for determining the date, hour
and location of its meetings. The length of each meeting shall not exceed
one hour except by majority vote of the Committee. Minutes of each
Committee meeting shall be prepared and filed for a period of at least one
year and shall be made available for review by Non-Compliance personnel of
the Division of Industrial Safety and Health of the Washington State
Department of Labor and Industries. A copy shall also be provided
promptly to the City Manager.
VI. EFFECTIVE DATE: This Administrative Order is effective immediately.
